Advance directives are written documents that give patients the opportunity to communicate their preferences regarding treatments they do or do not want to receive in case they become unable to make decisions. Commonly used pre-printed forms have different formats. Some offer space for patients to appoint a surrogate decision maker, and/or to determine future medical treatments and/or give a statement of personal values. So far it is unknown which forms GPs preferably use and why they decide to do so. 23 (...) semi-structured interviews with GPs were analysed using content analysis. Interviewees mainly use short templates and medium length templates with checkboxes to indicate patients’ preferences in regards to life prolonging measures. Especially when patients faced the progression of a disease, participants use the latter version. Only then, the interviewees remarked, patients are capable to rate concrete situations reliably. GPs also realize the importance of the verbal assessment of patients’ preferences; however they rarely keep a written form of the conversation. Some GPs hand out one or more templates and ask their patients to read and think about them at home with the option to talk to them about it later on, while others prefer their patients to fill them out alone at home. Regardless of template usage, most GPs emphasize that ADs require regular updates. GPs tend to see standardized advance directives mainly as a tool to start a conversation with their patients and to identify their real preferences and values. When the patient is still not facing the progression of an already existing disease it could be sufficient to only appoint a surrogate decision maker instead of creating a full AD. However, in all other situations, appointing a surrogate decision maker should be backed up by a written statement of a patient’s general values. Patients and their relatives should always have the opportunity to ask their GP for medical advice when drafting an AD. It is crucial to regularly verify and update existing ADs within the course of a disease. (shrink)

2006 wurde an der Washington University in St.Louis, USA, ein Seminar veranstaltet, das sich mit der Rezeption von Kelsens Lehre in verschiedenen Staaten befasste. Einige der Referenten stellten ihre Beiträge für diesen Band zur Verfügung.

GPs usually care for their patients for an extended period of time, therefore, requests to not only discontinue a patient’s treatment but to assist a patient in a suicide are likely to create intensely stressful situations for physicians. However, in order to ensure the best patient care possible, the competent communication about the option of physician assisted suicide as well as the assessment of the origin and sincerity of the request are very important. This is especially true, since patients’ requests (...) for PAS can also be an indicator for unmet needs or concerns. Twenty-three qualitative semi-structured interviews were conducted to in-depth explore this multifaceted, complex topic while enabling GPs to express possible difficulties when being asked for assistance. The analysis of the gathered data shows three main themes why GPs may find it difficult to professionally communicate about PAS: concerns for their own psychological well-being, conflicting personal values or their understanding of their professional role. In the discussion part of this paper we re-assess these different themes in order to ethically discuss and analyse how potential barriers to professional communication concerning PAS could be overcome. In this volume specialists of medieval music and philosophy put the medieval 'musica' into the context of ideas and institutions in which it existed. The significance of 'musica' cannot be understood from a modern point of view since 'music' does not match the medieval 'musica'.

"Ist der Patient ein Mensch?" is a collection of articles. Its central topic is the question if the object of modern medicine is still the human being or if just organs and tissues are treated by medical doctors? The drawing on the book cover shows a lung instead of a human patient lying in a hospital bed. The approaches to this topic are diverse and surprising. E.g. the surgeon Ankersmit argues that it is impossible for him to see the human (...) being in the body lying on the operation table because then he would be unable to perform his work professionally. Models of cognitive vulnerability claim that depressive symptoms arise as a result of an interaction between negative affect and cognitive reactions, in the form of dysfunctional attitudes and negative inferential style. We present a model that complements this approach by focusing on the appraisal processes that elicit and differentiate everyday episodes of emotional experience, arguing that individual differences in appraisal patterns can foster negative emotional experiences related to depression (e.g., sadness and despair). In particular, dispositional appraisal biases facilitating the elicitation (...) of these emotions more frequently and more intensely. This, in turn, is likely to have a negative influence on cognitive processing and emotion regulation in general. (shrink)

Recently, philosophers have come forth with approaches to chemistry based on its actual practice, imparting to it a proper aim and character of its own. These approaches add to the currently growing movement of pluralist philosophies of science. We draw on recent pluralist accounts from chemistry and analyse three notions from modern chemical practice and theory in terms of these accounts, in order to complement the so far more general pluralist approaches with specific evidence. Our survey reveals that the concept (...) of element indicates conceptual pluralisms in chemistry. that electronegativity illustrates methodological pluralism in determining one and the same concept, and that acidity is an instance of plurality hidden behind ‘one notion’. Electronegativity is a quantified, typical chemical concept, which correlates the ability of chemical species to attract electrons during their contact with other species with measurable quantities such as dissociation energies, dipole moments, ionic radii, ionization potentials, electron affinities and spectroscopic data. It is applied to the description and explanation of chemical polarity, reaction mechanisms, other concepts such as acidity and oxidation, the estimation of types of chemical compounds and periodicity. Although this concept is very successful and widely used, and in (...) spite of the fact that it is still subject to scientific investigations, neither a more than intuitive definition nor a generally accepted, logically clear and standardized quantification model has been developed. In the present work, electronegativity is presented and discussed with respect to its main conceptual and operational continuities and discontinuities. We try to analyze the epistemological status of electronegativity, conceived as a typical notion of chemical sciences. Under ‘epistemological status’ we subsume the issues of its reference, its historical persistence, and the relationship between its measurement and quantification. (shrink)

Images are objects of work in the laboratory. On its face, this work is achieved through talk Yet the talk attached to these images makes reference to other images, which are drawn from varcous environments. In this article, four such environments are identified: the domain of laboratory practice; the context of invisible physical reactions; the future image as it will appear in publication; and the domain of case precedents and reference scenarios from the field. The work of image analysis brings (...) the outside of the image into it and takes the inside out. Thus it can be seen that images are not just taken, they are designed and made. (shrink)

Immanuel Kant has built up a dualistic epistemology that seems to fit to the peculiarities of chemistry quite well. Friedrich Paneth used Kant’s concept and characterised simple and basic substances which refer to the empirical and to the transcendental world, respectively. This paper takes account of the Kantian influences in Paneth’s philosophy of chemistry, and discusses pertinent topics, like observables, atomism and realism.

Positive and negative feelings were central to the development of economics, especially in utility theory in classical economics. While neoclassical utility theory ignored feelings, behavioral economics more recently reintroduced feelings in utility theory. Beyond feelings, economic theorists use full-fledged specific emotions to explain behavior that otherwise could not be understood or they study emotions out of interest for the emotion itself. While some analyses display a strong overlap between psychological thinking and economic modelling, in most cases there is still a (...) large gap between economic and psychological approaches to emotion research.
